AC 120V Ground Fault

I have 2 6000xps off grid.  The other day a worker in my house bumped hot to equipment ground (ground fault), this did not clear its 15a 1P breaker but did code 19 the secondary inverter and jacked the voltage to 0v on “A” phase and 240v on “B” phase.  This voltage mess in turn burned up a branch power strip w/surge protection creating another ground fault causing same voltage mess.  Also a surge protected strip faulted a few months back causing same problem.

Does anyone know why the branch circuit breaker did not open (circuitry is all correct and compliant)?  More bothering is why did the inverters send 240v on one leg (“B”) phase!?!?
